Despite President Donald Trumps pledge to maintain a firewall between his his administration and his businesses by placing his sons in charge of the Trump empire, new reports have surfaced that Donald Jr. and Eric Trump are ramping up their engagement with the Republican Partys national political operation to discuss political strategy.

Trumps sons, along with Eric Trumps wife, Lara, met privately earlier this week with GOP leaders to share their concerns and outlook according to a Washington Post story detailing their visit to the Republican National Committees headquarters in Washington.

Their visit to the RNC raised red flags among two prominent Republicans who were cited by the Post wondering whether it was appropriate for the presidents sons, who run the Trump family real estate business, to be highly involved in discussing the partys strategy and resources.

Since their father took office, Donald Jr. and Eric Trump have fundraised for Republicans, rallied their fathers supporters in advance of his 2020 presidential bid, and defended their father on social media, while at the same time managing their fathers empire, according to a story in Politico.

But ethics experts told The Atlantic in a story published earlier this week that the steps Trump has taken so far to remove himself from the day-to-day operations of his organization has done little to resolve issues of conflict and influence.

The presidents plan to reduce spending ran into some conservative opposition, in addition to criticism from Democrats, for cuts to Medicaid and other programs

Donald Trumps proposal to reduce spending by $3.6tn, mostly by slashing antipoverty programs that provide social safety nets for the poor, ran into bipartisan opposition on Capitol Hill, where a number of Republican lawmakers rejected the cuts as draconian and nonstarters.

.

The presidents plan recommends $616bn in cuts to Medicaid, the government insurance programme for the poorest and many disabled Americans, while increasing border security spending by $2.6bn  including $1.6bn to begin construction on a wall along the US-Mexico border.

.

While fiscal conservatives welcomed the proposal, which aims to balance the budget by 2027, Republicans raised concerns that the plan would slice too deeply into programs that provide poor Americans access to healthcare, food stamps and student loans, setting the stage for a showdown over budget priorities.

.

The cuts are draconian, Kentucky representative Harold Rogers, a powerful member of the House Appropriations Committee, said of the proposed cuts to Medicaid.

.

Representative Mark Meadows, the chairman of the arch-conservative Freedom Caucus, praised the budget as conservative and said the White House presented a plan that fundamentally could be implemented. Though Meadows said he was encouraged by budget reductions to welfare programs, he could not get behind stripping funding from Meals on Wheels, a program that provides meal assistance for senior citizens that the presidents budget chief, Mick Mulvaney, dismissed in March as just not showing any results.

Arizona senator John McCain called the proposal dead on arrival in a statement, and knocked the $603m defense budget request inadequate to the challenges we face.

.

Senate majority leader Mitch McConnell told reporters, The presidents budget, as we all know, is a recommendation. Well be taking into account what the president is recommending but it will not be determinative in every respect.

.

Trumps package of spending cuts and tax breaks assumes an economic growth rate of 3%, which many analysts have dismissed as improbable. The Congressional Budget Office forecast under current policy is 1.9%.

.

Presidential budgets are wish lists the White House sends to Congress. There was never a chance it would pass through untouched.

.

Clearly Congress will take that budget, and then work on our own budget, which is the case every single year, but at least we now have common objectives, House Speaker Paul Ryan told reporters.

I visited the century-old library that sits atop Needle Park in Kensington because Id heard its staff was the first in the city to learn how to administer the lifesaving overdose antidote Narcan.

They have been using the spray so often that they can tell the type of overdose simply by the sound coming from the lavatory: Heroin victims slide sluggishly into unconsciousness, the librarians have found, while victims of deadly fentanyl collapse instantly, with a thud that resonates through the entire building, which is called the McPherson Square Branch.

.

Since the opioid crisis began surging throughout the country last summer, the library staff has noticed new settlers on their lawn: drug tourists, they call them. Young people traveling from across the country for Phillys pure heroin and the high said to be better than anywhere else.

Overdose drills

.

For nearly 30 years, Judi Moore has worked as branch manager and childrens librarian at McPherson, which serves the drug-ravaged neighborhood around Kensington and Indiana Avenues. Her desk offers a gunpoint vantage.

.

Until last year, she recalls just one overdose in the library. Then heroin exploded. Since then, there have been four overdoses in her building. None has been fatal.

.

There was the man who shot up in the adult reading room and slid to the floor. And the man who thudded to the bathroom tile and was blue and shaking and gasping for breath. There was that Saturday late last year when McPherson called 911 five times in one day.

.

As the library overdoses mounted, the soft-spoken Moore, whom the kids call Miss Judi, took actions, small and large.

.

First, she posted a bathroom monitor  and accompanying rules she hoped would pose obstacles to those whod use the bathroom to get high:

Adults must leave a library card or ID at the front desk, the sign reads. And bathroom use is limited to three to five minutes. After that, a guard knocks.

.

While other libraries practice fire drills, McPherson began overdose drills:

.

Who stays with the victim? Who calls 911? Who ushers out the kids? Who waits for an ambulance?

.

The steps helped, but then the tourists swarmed.

.

White kids, mostly. And the addresses listed on the driver’s licenses offer evidence of the opioid crisis reach: Jersey. Delaware. Ohio. South Carolina. Alabama. Arkansas. Nevada.

.

With its after-school lunches and arts and STEM programming, the library has long sought to be a safe learning space. McPherson  set to celebrate its 100th anniversary Saturday — has the highest programming-attendance rate of any city branch. And is also among the top for WiFi use. Kids come here to use computers they dont have at home.

And the park had been getting nicer in recent years, with a new playground and lighting.

.

But now, more than ever, children must watch their steps for needles.

.

Teddy Hackett, a volunteer who scours the grass daily, says he collects double the needles he did only a few years ago.

.

On Thursday, he collected 107, showing his bucket. He even finds them on the jungle gym.

Republican leaders in Congress say they will go their own way on spending and largely ignore the budget President Trump sent to Congress on Tuesday, underlining a divide between the White House and his congressional allies on spending issues.

.

Senate Majority Leader Mitch McConnell (R-Ky.) downplayed the significance of setting Trumps budget aside as he told reporters that he will soon begin negotiations with Democrats on the top-line numbers for discretionary spending bills.

.

Asked about Trumps budget, McConnell said its traditional for Congress to discard much of a presidents blueprint.

.

Well be taking into account what the president is recommending, but it will not be determinative in every respect, he said. I didnt engage in a ringing endorsement of President [George W.] Bushs budgets either.
Others in the Senate GOP conference immediately distanced themselves from Trumps budget.

Sen. Shelley Moore Capito (R-W.Va.) said the plan cuts too close to the bone, while Sen. Dean Heller (R-Nev.) called it anti-Nevada, citing cuts to important public lands programs.

.

Senate Health Committee Chairman Lamar Alexander (R-Tenn.) has previously warned that not much more can be cut from discretionary spending accounts, which cover popular programs such as medical research at the National Institutes of Health.

.

Trumps proposed cuts to Medicaid were making some Republicans nervous.

.

The fiscal 2018 budget assumes that the House GOPs legislation to repeal and replace ObamaCare will become law, which would cut Medicaid by $839 billion. Trumps budget then proposes an additional $610 billion in cuts.

.

I have a major problem with that, said Capito, who has voiced her concerns about steep Medicaid cuts to the 13-member working group that McConnell has convened to negotiate the Senate healthcare reform bill.

.

Democrats pounced on Trumps plan as brutal and cruel and predicted that spending cuts targeting programs helping poor and vulnerable families would boomerang on Trump and the GOP.

.

The irony of the Trump budget is that it hurts many of the people who supported him most in the campaign. Thats the great irony of the budget. When you add it all up, the Trump budget is a comic book villain-bad budget, said Senate Democratic Leader Charles Schumer (N.Y.).

As with previous presidents budgets, President Trumps budget released yesterday is more of a gesture than anything else. The media will look to it to gauge what the presidents priorities are while members of Congress will identify sections they find useful and discount the others. But this year, I think the GOPs response, or lack thereof, to Trumps budget is illustrative of a problem no one is quite comfortable discussing Republicans have not come to terms with what a Republican budget should look like. The Trump budget is a wake-up call for Republicans. There is no President Barack Obama to blame for budget stalemates and out-of-control spending.

So, will we be Obama-lite on government spending? Do we want to maintain the status quo on entitlements, or will we seriously reduce the size and scope of the federal bureaucracy? Do we just want to limit and manage the lefts usual criticism that Republicans are interested only in denying benefits to widows, orphans, etc., or are we prepared to make difficult but necessary spending reductions and then sell the budget to the public and run on it in 2018? It may be considered political suicide to reduce spending and balance the budget within 10 years, but isnt it economic suicide if we continue on the present course?

With former congressman Mick Mulvaney serving as the architect of the presidents budget and director of the Office of Management and Budget, the world knows that Trump is prepared to reduce spending, balance the budget and honestly change the trajectory of government. The budget may have been a gesture, but it was a meaningful one nonetheless.

Despite the bleak outlook offered by Democrats and anti-Trump political pundits who oppose almost any spending cuts, the fact remains that Republicans must take ownership of this budget and follow through on years of promises to make the tough choices that so many of our representatives in Congress campaigned on.

.

The Trump budget scales back the size and scope of the federal government and reduces or eliminates funding for programs that are wasteful or, in some instances, more appropriate for the states to handle. Those are top priorities for any member of the Republican Party, right?

When Salama Ali started investigating the disappearance of two younger brothers last year she made an awful discovery – not only were radicalised young Kenyan men leaving to join the al-Shabab militants in neighbouring Somalia, but women were being seized and trafficked by the group as sex slaves.

Salama’s search for information about her brothers had to be carried out quietly and confidentially, as any hint of a connection with the al-Qaeda-linked al-Shabab can arouse the suspicion of the security forces.

.

So she met discreetly with other women in Mombasa and the surrounding area, sharing stories and seeking information about male relatives who had vanished.

.

“We discovered there were lots of us,” Salama says.

.

But Salama also uncovered something very different – stories of women who had been taken to Somalia against their will.

.

The women were both young and old, from Christian and Muslim communities, from Mombasa and other parts of Kenya’s coastal region. They were usually promised high-paid work in another town or abroad, and then kidnapped.

.

Last September Salama trained as a counsellor and set up a secret support group for returning women. Word spread and soon women began seeking her out and asking to join the group.

.

Some arrived with babies, she says, some with HIV, and some with mental illness caused by their experiences. All are terrified to speak openly, because of the risk of being mistakenly identified as an al-Shabab sympathiser.

.

In a dark room with the curtains drawn, I meet this extraordinary group of women, who have a story that has never been told.

.

“Men used to come and have sex with me – I can’t tell you the number,” says one, shaking her head as she recounts her ordeal. “For those three years, every man was coming to sleep with me.”

.

“They’d bring two or three men for each woman every night,” says another. “We would be raped repeatedly.”

Some women were forced to become the “wives” of al-Shabab militants, it appears, while others were held as slaves in a brothel.

Al-Shabab is fighting to create a fundamentalist Islamic state in Somalia and has launched attacks on neighbouring countries, which have all sent troops to fight them as part of an African Union force.

One new member of Salama’s group, Faith, has only recently escaped captivity.

.

She was 16 when she was approached by an elderly couple and offered a job in Malindi, further up the coast. Desperate for work, the next day she boarded a bus with 14 other passengers and all were given drugged water to drink.

“When we regained consciousness, there were two men inside the room,” Faith says. “They blindfolded us with black scarves. They raped us in that room.”

Drugged again, Faith woke up in a small clearing in a dark forest and was told she would be killed if she tried to escape.

.

Terrified, she spent the next three years alone cooking for a group of Somali men “with long long beards”.

.

She had also become pregnant, as a result of being raped, and had to deliver her own child alone in the forest.

When young athletes sustain concussions, they are typically told to rest until all symptoms disappear. That means no physical activity, reading, screen time or friends, and little light exposure, for multiple days and, in severe cases, weeks.

Restricting all forms of activity after a concussion is known as cocooning. But now new guidelines, written by an international panel of concussion experts and published this month in The British Journal of Sports Medicine, question that practice. Instead of cocooning, the new guidelines suggest that most young athletes should be encouraged to start being physically active within a day or two after the injury.

.

The brain benefits from movement and exercise, including after a concussion, says Dr. John Leddy, a professor of orthopedics at the Jacobs School of Medicine and Biomedical Sciences at the University at Buffalo, and one of the co-authors of the new guidelines.

.

There has long been controversy, of course, about the best ways to identify and treat sports-related concussions. Twenty years ago, athletes who banged their heads during play were allowed to remain in the practice or game, even if they stumbled, seemed disoriented, or were seeing stars. Little was known then about any possible immediate or long-term consequences from head trauma during sports or about the best responses on the sidelines and afterward.

Since then, mounting evidence has indicated that sports-related concussions are not benign and require appropriate treatment. The question has been what these appropriate treatments should be.

.

In the early 2000s, dozens of the worlds premier experts on sports-related concussions started meeting to review studies about concussions, with plans to issue a consensus set of guidelines on how best to identify and deal with the condition.

.

The panel, called the Concussion in Sport Group, does not make formal clinical practice guidelines. But the groups findings do represent the latest thinking about sports concussions by the worlds experts, based on the newest published science, says Dr. Leddy, who is also medical director of the Buffalo Concussion Management Clinic.

.

In its 2012 guidelines the Concussion in Sport Group recommended broadly that if an athlete of any age was found to have a sports-related concussion, he or she should rest as completely as possible, remaining in a darkened room with little visual or physical stimuli, until all symptoms had gone away and did not return once the athlete began easing back into normal activities, which could be a week or more.

This approach was thought to promote recovery by minimizing brain energy demands following concussion, the authors write in the new statement.

.

But since then, a number of studies in animals and people with diagnosed concussions have indicated that prolonged physical rest may actually delay the brains recovery.

.

So this month, the group published a new set of guidelines that significantly revise the recommendation for physical rest. Now the advice is that after a concussion an the athlete should remain quiet for 24 to 48 hours, but then should begin to get up and move.
