I enjoy this because it combines three great Trump tropes into one dense body, like a super-heavy collapsed star made of bullshit.

.

1. “We have the votes.” They don’t. POTUS keeps saying that Graham-Cassidy failed only because there was a Republican senator in the hospital when it was being voted on. The senator in question wasn’t in the hospital, but the bill wouldn’t have passed even if he had been because of other GOP opposition.

.

2. Random, spontaneous claims about scheduling. You may recall that during the campaign Trump promised to repeal Obamacare on his first day in office. That … didn’t happen. During the transition, in January 2017, he said he wanted repeal to take place within “weeks” after inauguration. That also didn’t happen. Now it’s going to be done by April of 2018. Maybe that will happen! (Recall also that Trump keeps saying he’s going to get his vaunted border wall done “ahead of schedule” even though it hasn’t yet been so much as designed or budgeted for.)

.

3. An extremely unpopular idea being revived over and over and over and over and over again because the president’s only real guiding principle is that he wants to undo everything his predecessor did. A spring 2018 revisiting of Graham-Cassidy would be, by my count, the sixth distinct Affordable Care Act repeal effort. (There was the first House bill, the House bill that passed, the first Senate bill, the “skinny repeal” bill, and the first Graham-Cassidy bill.) None of these bills have polled well, and they’ve sucked up all the oxygen in Washington, D.C. so that nothing else has gotten done. But: Obama bad!

The story of Donald Trump and health coverage rightly starts with the story of Donald Trump and William Trump.

.

William was born in 1999, fighting for his life. He suffered seizures, and his breathing stopped twice. William’s medical bills were enormous. That’s when Donald tried to remove the infant from the Trump family health plan.

.

Why? Retaliation against his nephew (William’s father) for challenging the will of Fred Sr. It seems the Trump patriarch had left the family of one son  Donald’s deceased brother, Fred Jr.  largely out of his estate. Any part Donald may have played in nudging the elderly Fred Sr. to do that is not public knowledge. What’s clear is that with his brother basically cut from the will, Donald inherited many more millions.

.

But the family lawyer ruled that no, you can’t throw baby William off the family’s health plan. William survived, though he later developed cerebral palsy.

.

My point is to note the president’s indifference to questions of life and death when dollars enter the picture. Here he actually held his own grandnephew’s life hostage in a fight over money.

.

Which brings us to Trump‘s diabolical crusade to blow up the Affordable Care Act. The Republican Congress failed to kill Obamacare for him, so he’s now single-handedly destabilizing the health exchanges through various executive acts. Should they collapse, he’ll say over the wreckage, “See? I told you it was a disaster.”

.

Trump‘s latest, and most extreme, move is to take away $7 billion in subsidies that help 7 million low-income people afford health insurance. The result would be as follows: The sick desperate for coverage would bear the freight, while the healthy would have less incentive to buy into the exchanges. Premiums for the popular plans would spike by 25 percent, the Congressional Budget Office estimates.

.

Heaven knows Republican lawmakers tried to destroy Obamacare on their own. They couldn’t because nearly the entire medical establishment called for saving the program, and huge numbers of their constituents depend on it.

This year, Washington has wasted months in an often-misguided debate over repealing the Affordable Care Act (ACA). And, while this debate has raged within one political party, two important programs have been forgotten: the Childrens Health Insurance Program (CHIP) has expired and the Medicaid disproportionate share hospital (DSH) program was left to be slashed at the beginning of this month.

.

Both programs have traditionally enjoyed very broad bipartisan support. I partnered with a Republican colleague in our legislature to fund Pennsylvanias first-in-the-nation CHIP program. And, in 2015, the CHIP program was reauthorized with language that prevented cuts to DSH payments by a decisive margin of 92-8 in the Senate and 392-37 in the House.

The DSH program supports safety net hospitals that serve a high proportion of low-income patients whose care is not entirely covered by third-party payers (such as Medicaid, Medicare, and CHIP). The DSH program provides payments to those hospitals to offset the uncovered costs. Its a critical safety net program for hospitals that provide an invaluable service to people who need it the most.

The architects of the ACA included provisions in the law that cut DSH payments over time to compensate for what they expected to be reduced demand as more people were insured. Those cuts were also intended to help pay for the laws coverage expansion.

.

The reality, though, is that the proposed coverage expansions and projected reduction in the number of un- or under-insured patients did not happen the way Congress originally intended. The Supreme Court gave states the option to refuse to participate in expansion of Medicaid and the Trump administration has threatened to withhold health care coverage payments, and in fact has already cut the CSR payments, which undergird the ACA.

The result is that DSH program cuts were still on track despite the ongoing need for DSH resources. And, Congress has failed to reauthorize CHIP over squabbles about how to pay for it and the distraction of the failed efforts to repeal the ACA.

.

That puts us in a tough spot in Philadelphia and something must be done.

.

As I have noted, the deadline has already come and gone. According to the most recent Kaiser Family Foundations State Health Facts, Pennsylvanias current DSH allotment is $616.27 million. Cuts to the program will reduce payments by $121.03 million, a 19.6 percent reduction. And, we have 300,000 kids enrolled in CHIP which will soon run out of money to pay for their care.

Why bother trying to be healthy? We all have to die of something, right?

.

In part this is true. Regardless of our discomfort with death, we all have to die sometime, and we all have to die from something. However, this is where the truth ends.

.

Today in Australia, the leading causes of death are mostly preventable  or at least can be significantly delayed. Factors like poor diet and tobacco drive ailments including heart disease, stroke, diabetes (type 2), lung disease and cancers. And when you look more deeply at what ill-health brings, its not just death that makes the strongest case for getting out of bed.

Youll die later

Youll be healthier, longer

Youll feel better in the meantime

Youll save more and spend less

Your kids will be healthier, and maybe even their kids

The bigger picture

.

Living a healthier lifestyle is about making small, possible, simple and sustainable changes like taking the stairs instead of the lift  not totally rethinking the way you live.

.

But while there are many compelling reasons to be healthy, personal discipline alone will never solve our chronic disease and obesity epidemics. If neighbourhoods lack safe spaces for kids to run, or fresh food is inaccessible and unaffordable, good intentions will not get you very far. Government policies need to make health easier, even preferred.

.

In the meantime, look to friends for motivation; your family to share and support a healthy diet; apps that map and commend the exercise you do; and your GP and important services like Quitline for assistance with alcohol reduction and smoking cessation.

Should a Bibliography be Published in a Book or on the Internet?
Some Thoughts

The question was raised on a discussion group as to the best publisher to
use for the publication of a disabilities related bibliography.

I made the observations below regarding this query.

.

.

This depends on the outcome one is looking for by sharing this content.
Publishing a bibliography is not the best way of putting this content
out there.  Bibliographies are out of date rapidly as new books and
articles emerge.  In this age when academic libraries have hundreds of
databases, the motivation to purchase bibliographies is limited, so they
are unlikely to be a money making resource.  Print books cannot be
updated, unless one publishes new editions and these need to be spread
over time.

.

On the other hand one can publish annual collections of publications on a
topic covered in a bibliography on a website, as a blog post or as a post
to a discussion group each time and have an index to each years edition on
a website.  Google Sites are free to users and websites can have many
pages and only require ownership of a Gmail account as the login for
access to Google Sites.  One can arrange the website alphabetically with
different web pages for each letter or for groups of letters of the
alphabet.  One could also arrange web pages for each year,s sources and
produce a chronogical document rather than an alphabetical citation
source.

.

One can back up this content on Blogs and/or discussion groups.  WordPress
blogs, Google Sites, and Groups.IO discussion group posts can all be
edited so one can add content whenever one wishes to update.  I cannot
remember the last time I purchased a bibliography for the library
collection where I work.  Also, in my collection development work, book
purchases are overwhelmingly for print books.  In my subject fields, about
ten percent of the books I view citations for are available online and of
those five percent are anywhere from double the price of the paperback
edition to four or five or six or more times the price of the paperback.
In print books, one does not have a search engine box for content within
the book to view the occurances within the book of words and phrases, so
those wishing citations on a very specific topic may have a large task to
find such content within a book unless the author has done an exceptional
job of organizing and indexing that content.

There is a huge body of biblographic sources available on the internet for
most broad topics one can think of, not to mention the bibliographies
included in books and dissertations about a specific subject.

A high proportion of adults in England are insufficiently active.1 The workplace is an important setting for promoting physical activity with potential to reach a high proportion of adults in employment (74.8%).2 3 The Workplace Challenge aims to increase sport and physical activity participation in inactive employees. The programme is led and managed nationally by the County Sports Partnership Network (CSP Network); and delivered locally by County Sports Partnerships (CSPs), National Governing Bodies for Sport (NGBs) and other local and national partners.

Workplace and employee recruitment

.

A variety of strategies are used to market the programme to workplaces and employees. CSPs telephone, email and visit workplaces, attend local business network events, publish in business news journals, use social media, publish press releases, and distribute materials and newsletters using local networks and the local CSP website. Many participants are recruited through word of mouth or workplace promotional activities.

Programme components

.

The programme includes four key components underpinned by selected behaviour change techniques4 (table 1). Participants can log their activities on the website/app (activity type, duration and intensity), set personal goals and track their progress towards achieving 150?min of moderate-vigorous physical activity (MVPA) each week. Regular national online activity logging challenges are implemented. For example, the 8-week active lunch challenge (January/February 2017) encouraged employees to do an activity during their lunch break. Points were awarded for each activity recorded, and leader boards enabled participants to assess their progress against others, either as individuals or as part of a team. Other shorter challenges are offered locally, for example, a 4-week active travel challenge. CSPs and NGBs organise local events and competitions, for example, table tennis competition, social summer cricket, rowing taster session, learn to ski session, corporate football tournament. Finally, workplace champions are recruited and trained to enable them to organise activities within their own workplaces, for example, taster sessions, internal walking challenges or interdepartmental sports competitions.
