Concussion in the National Football League (NFL) remains an important issue. An initial description of the injury epidemiology involved 6 years from 1996 to 2001.

Hypothesis:

The increased attention to concussions may have resulted in team physicians being more conservative in treating players in recent years.

.

Study Design:

.

Two consecutive 6-year periods (1996-2001 and 2002-2007) were compared to determine changes in the circumstances associated with the injury, the patterns of signs and symptoms, and the players time loss from participation in the NFL.

.

Methods:

.

During 2002-2007, concussions were recorded by NFL team physicians and athletic trainers using the same standardized reporting form used from 1996 to 2001. Player position, type of play, concussion signs and symptoms, loss of consciousness, and medical action taken were recorded.

.

Results:

.

There were 0.38 documented concussions per NFL game in 2002-20077.6% lower than the 0.42 in the earlier period (1996-2001). The injury rate was lower in quarterbacks and wide receivers but significantly higher in tight ends during the second 6 years. The most frequent symptoms were headaches and dizziness; the most common signs were problems with information processing and immediate recall. During 2002-2007, a significantly lower fraction of concussed players returned to the same game, and more were removed from play. Most concussed players (83.5%) returned to play in < 7 days; the percentage decreased to 57.4% with loss of consciousness. The number of players returning in < 7 days was 8% lower during 2002-2007 and 25% lower for those with loss of consciousness.

Conclusion:

.

The most recent 6 years of NFL concussion data show a remarkable similarity to the earlier period. However, there was a significant decrease in the percentage of players returning to the same game, and players were held out of play longer.

.

Clinical Relevance:

There was a more conservative management of concussion in NFL players from 2002 to 2007 even though the clinical signs and symptoms remained similar to the earlier 6-year period.

There are a number of possible explanations for the changes in the RTP data and the incidence of concussion during the second 6-year period. Multiple factors may have played a role in causing these differences, including the efforts by the NFL, the NFL Players Association, NFL team athletic trainers and physicians, and safety equipment manufacturers to ameliorate the effects of head injury on NFL players.

Abstract

Ketorolac tromethamine (Toradol) is a non-steroidal anti-inflammatory drug that has potent analgesic and anti-inflammatory properties. It can be administered orally, intravenously, intramuscularly, or via a nasal route. Ketorolac injections have been used for several years in the National Football League (NFL), in both the oral and injectable forms, to treat musculoskeletal injuries and to prevent post-game soreness. In an attempt to determine the appropriate use of this medication in NFL players, the NFL Team Physician Society appointed a Task Force to consider the best available evidence as to how ketorolac should be used for pain management in professional football players. These treatment recommendations were established based on the available medical literature taking into consideration the pharmacokinetic properties of ketorolac, its accepted indications and contraindications, and the unique clinical challenges of the NFL. The Task Force recommended that 1) ketorolac should only be administered under the direct supervision and order of a team physician; 2) ketorolac should not be used prophylactically as a means of reducing anticipated pain either during or after participation in NFL games or practices and should be limited to those players diagnosed with an injury or condition and listed on the teams injury report; 3) ketorolac should be given in the lowest effective therapeutic dose and should not be used in any form for more than 5 days; 4) ketorolac should be given in its oral preparation under typical circumstances; 5) ketorolac should not be taken concurrently with other NSAIDs or by those players with a history of allergic reaction to ketorolac, other NSAIDs or aspirin; and 6) ketorolac should not be used by a player with a history of significant gastrointestinal bleeding, renal compromise, or a past history of complications related to NSAIDs.

Beginning with the introduction of ibuprofen in the 1950s, nonsteroidal anti-inflammatory drugs (NSAIDs) have been prescribed for athletic injuries in an attempt to blunt the bodys inflammatory response to injury, control pain, and aid in the return to sports.26 There are a variety of both prescription and nonprescription medications in this class of drugs. Among athletes using prescription medication, NSAIDs have been found to be among the most frequently prescribed (8.1%).1 Unfortunately, these agents are not without complications (see the following). Adverse events were reported in 20% of athletes using NSAIDs for a variety of musculoskeletal complaints.1

.

Athletes may even take NSAIDs as a preventive measure. For example, during the 2000 Olympic Games in Sydney, Canadian athletes used NSAIDs more than any other medication.17 Similarly, a survey of American football players showed that 1 out of 7 high school athletes took NSAIDs daily and that 29% of college athletes took them as a preventive measure on the day of a game.30 Warner et al31 found a similar occurrence in their study of athletes; independent of their analgesic effect, the athletes mentioned a potential performance improvement to justify taking these medications.

Ketorolac tromethamine (Toradol) is an NSAID that has potent analgesic and anti-inflammatory properties. It can be administered orally, intravenously, intramuscularly, or via a nasal route. Ketorolac has been used principally for its analgesic properties following acute strains and sprains, overuse injuries, and as an adjunct to narcotic medication following surgery. Ketorolac injections have been used for several years in the National Football League (NFL), in both the oral and injectable forms, to treat musculoskeletal injuries and to prevent postgame soreness. The only study, to date, examining the prevalence of ketorolac use in professional sports was performed by Tokish et al in 2002.29 These authors investigated the use of injectable ketorolac in NFL teams during the 2001 season. Their study revealed that 28 of the 30 teams that responded to their survey used intramuscular ketorolac. Game-day usage was reported at 93% with pain relief of 1 to 2 days noted in 50% to 75% of players. There were 6 adverse reactions reported, including 4 muscle injuries, 1 gastrointestinal (GI) complaint, and 1 case of postinjection soreness. Anecdotally, some NFL medical staffs felt that ketorolac injections were considered more powerful than other NSAIDs because of the route of administration in that many players felt that getting a shot was an intrinsic sign that they were getting a more powerful medicine.29 Overall, these authors found ketorolac to be safe and effective when used in the pregame setting of the NFL. However, they were careful to recommend further study to develop standardized guidelines for ketorolac use in athletes to protect both players and medical staffs from potential complications. Since the publication of this study, it is widely believed by NFL team physicians that the use of ketorolac has increased in prevalence not only in the NFL but also in NCAA Division I football. However, we are unaware of any objective documentation proving this hypothesis.

Sports medicine specialists often use injections to deliver the intended medication directly to the site of injury (anesthetic or corticosteroid) or to provide a medication when oral dosing is not possible. Sterile preparation may reduce the risk of infection, but this can be challenging in the game-day training room or hotel setting. In addition, injections clearly pose a risk of bleeding and injury to adjacent structures. As with the administration of any other form of invasive treatment, informed consent has been recommended prior to administering injections to athletes.27

.

Injections are typically perceived as a more aggressive form of treatment that has recently garnered attention from the lay media as a result of ketorolac injections used prior to competition in the NFL. Injection of medication to treat sports-related conditions is not unheard of, but there is no evidence of increased effectiveness owing to this route of administration. Orchard24 published the results of a case series involving professional Australian Rules football and rugby players treated with local anesthetic injections to facilitate a quicker return to play. This study showed an 8% complication rate associated with the use of injections in these players and was an important first step in trying to adequately define the risks and complications associated with injections following athletic injuries. Public perception of team physicians administering various therapeutic (or pain killing) injections to allow athletes to return to competition likely varies depending on the level of competition and public visibility of the athlete. Moreover, the literature is deficient in terms of the ethical considerations implicit with the administration of injectable medications in the athletic setting solely for the athlete to return to competition.

This week, a comprehensive study published in the Lancet revealed that the number of obese children, aged five to 19, worldwide has skyrocketed tenfold over the past 40 years.

.

The study, led by the Imperial College of London in collaboration with the World Health Organization, involved height and weight data on nearly 130 million people. While childhood obesity rates are on the rise in low- and middle-income countries, the rise appears to have levelled off, at least temporarily, in high-income countries such as Canada.

.

Good news perhaps, but our national childhood obesity numbers remain dismal and alarming. According to Statistics Canada, in 2015 nearly 15 per cent of boys and 10 per cent of girls aged five to 17 were considered obese, numbers that have nearly tripled since the 1970s.

.

Kids with obesity are at a higher risk for asthma, sleep apnea, type 2 diabetes and high blood pressure. They’re also more likely to be bullied than their peers who are at a healthy weight.

.

Obesity in childhood also increases the likelihood of becoming an obese adult and developing the health problems that come with it diabetes, heart disease and cancer.

.

At about 3 p.m. on Friday, February 3, Tim Piazza, a sophomore at Penn State University, arrived at Hershey Medical Center by helicopter. Eighteen hours earlier, he had been in the kind of raging good health that only teenagers enjoy. He was a handsome, redheaded kid with a shy smile, a hometown girlfriend, and a family who loved him very much. Now he had a lacerated spleen, an abdomen full of blood, and multiple traumatic brain injuries. He had fallen down a flight of stairs during a hazing event at his fraternity, Beta Theta Pi, but the members had waited nearly 12 hours before calling 911, relenting only when their pledge looked fucking dead. Tim underwent surgery shortly after arriving at Hershey, but it was too late. He died early the next morning.

Every year or so brings another such death, another healthy young college man a victim of hazing at the hands of one of the nations storied social fraternities. And with each new death, the various stakeholders perform in ways that are so ritualized, its almost as though they are completing the second half of the same hazing rite that killed the boy.

.

The fraternity enters a period of reflection; it may appoint a blue-ribbon panel. It will announce reforms that look significant to anyone outside the system, but that are essentially cosmetic. Its most dramatic act will be to shut down the chapter, and the house will stand empty for a time, its legend growing ever more thrilling to students who walk past and talk of a fraternity so off the chain that it killed a guy. In short order it will recolonize on the campus, and in a few years the house will be back in business.

The president of the college or university where the tragedy occurred will make bold statements about ensuring there is never another fraternity death at his institution. But he knowsor will soon discoverthat fraternity executives do not serve at the pleasure of college presidents. He will be forced into announcing his own set of limp reforms. He may ban the fraternity from campus, but since the fraternity will have probably closed the chapter already, he will be revealed as weak.

The media will feast on the story, which provides an excuse to pay an unwarranted amount of attention to something viewers are always interested in: the death of a relatively affluent white suburban kid. Because the culprits are also relatively affluent white suburban kids, there is no need to fear pandering to the racial bias that favors stories about this type of victim. The story is ultimately about the callousness and even cruelty of white men.

The grieving parents will appear on television. In their anger and sorrow, they will hope to press criminal charges. Usually they will also sue the fraternity, at which point they will discover how thoroughly these organizations have indemnified themselves against culpability in such deaths. The parents will try to turn their grief into meaningful purpose, but they will discover how intractable a system they are up against, and how draining the process of chipping away at it is. They will be worn down by the endless civil case that forces them to relive their sons passing over and over. The ritual will begin to slow down, but then a brand-new pair of parentsfilled with the energy and outrage of early griefwill emerge, and the cycle will begin again.

The even tougher steps needed to make the island more self-sustaining are well-known, but have been found difficult and left untried, said Anthony Stevens-Arroyo, a native of Philadelphias Puerto Rican diaspora whose long career as priest, professor and family man included testimony on Puerto Ricos future before the United Nations Decolonization Committee (starting in 1978) and the U.S. House of Representatives (1990s):

Puerto Rico needs to eliminate its dependency on imported sources of energy and stop relying on U.S. fuel imports, Stevens-Arroyo told me. Solar-power companies are already rushing panels and batteries to the island in hopes of restoring the wrecked electrical grid.

The island faces the same problems as other Caribbean islands and needs to trade and cooperate with them, not just depend on U.S. aid, debt, and fickle corporate investment.

If the U.S. under Trump cant or wont pay for reconstruction and public services, Puerto Rico should be recognized, not as an independent country a 19th-century concept that few Puerto Ricans support but as an autonomous political entity that has a right to representation within the U.N., like the Palestinians. This would make economic development and trade arrangements with the global community negotiable by Puerto Ricos elected government.

By contrast, at present the U.S. Congress governs the colony, he said. Though there is still an elected governor and legislator, fiscal power lies with the seven-member PROMESA oversight board, which favors bankers. With United Nations status, Puerto Rico would more likely qualify for participation with the World Bank, UNESCO, and other development grant and loan funders, subject to the fiscal restraints they place on borrower governments.
